We are seeking a dedicated and experienced Case Management Nurse Manager (RN – Complex Case Manager) to join our team. This role is pivotal in providing case management services for medically complex inpatients, including those with significant medical conditions, socio-economic challenges, and mental health co-morbidities.
The Nurse Manager will lead efforts to help patients achieve optimal health outcomes and independence in managing their care. This position requires a strong foundation in comprehensive case management principles, patient-centered care, and culturally sensitive coordination.
Responsibilities:Patient Assessment & Care Planning
- Assess physical, functional, social, psychological, environmental, learning, and financial needs of patients.
- Identify problems, set prioritized goals, and design interventions aligned with patient/caregiver preferences.
- Develop individualized inpatient care plans with objectives, actions, and measurable outcomes.
- Evaluate patient progress, reassess goals as health status changes, and troubleshoot barriers to care.
Care Coordination & Advocacy
- Implement and monitor care plans, ensuring effectiveness and appropriateness of services.
- Communicate with physicians, interdisciplinary teams, and caregivers to coordinate care.
- Act as a patient advocate, addressing barriers to care and promoting self-management.
- Facilitate referrals to community resources, coordinate discharge services, and prevent duplication of services.
Patient & Family Engagement
- Utilize motivational interviewing to build patient engagement in care plans.
- Provide education, information, and support to patients and families regarding care goals.
- Promote chronic disease management, preventive health initiatives, and health literacy.
- Encourage patient and family responsibility in achieving care outcomes.
Documentation & Compliance
- Maintain accurate patient records and confidentiality in accordance with policy.
- Document all relevant information following departmental guidelines.
- Participate in case reviews (prospective, concurrent, retrospective) and performance improvement projects.
- Uphold organizational mission, vision, values, and performance standards.
Program Development & Professional Growth
- Assist with collection, analysis, and benchmarking of utilization data.
- Collaborate in developing protocols and guidelines for patient care management.
- Engage in professional development to stay current with case management practices.
- Minimum 3–5 years of case management experience , preferably with complex patient populations.
- Strong knowledge of comprehensive case management principles .
- Excellent communication, coaching, and motivational skills.
- Ability to work collaboratively with patients, families, and interdisciplinary teams.
- Current RN license in Delaware or compact state
- Bachelors degree in nursing or related field OR AND with 5 years of case management experience with BSN completion within 5 years of hire required
