The MSHO RN Care Coordinator is a Registered Nurse responsible for managing and coordinating care for dual-eligible Minnesota Health Care Program members through assessments, patient education, and resource connection to improve health outcomes. The role requires collaboration with an interdisciplinary team, conducting both remote and in-person home visits in the Detroit Lakes, MN area. Candidates must hold an active RN license, relevant certifications, and possess care coordination experience to support patient transitions and close gaps in care.
This candidate will need to be in the Detroit Lakes, MN area so they can visit patients in the Detroit Lakes area.

Education Requirements:
- Graduate of 2- or 4-year Registered Nursing Program
Required Qualifications:
- 2 or more years of experience in care coordination or case/care management
- Must have reliable transportation
Preferred Qualifications:
- Certified Case Manager
- 5 or more years of experience in care coordination or case/care management
Certifications/Licensure:
- Current Registered Nurse License in the State of Minnesota
- Current MnCHOICES Assessor certification (or ability to obtain within 30 days of hire)
- Current Basic Life Support certification (or ability to obtain within 90 days of hire)
- Must have a valid driver’s license
