A well-established Program of All-Inclusive Care for the Elderly (PACE) is seeking a Locum Nurse Practitioner (NP) to provide primary medical coverage for an ongoing assignment, with the potential to extend. This is a rewarding opportunity to work with a collaborative team delivering compassionate care to frail and elderly participants in a variety of settings.

What You ll Do:
Provide primary care in collaboration with physicians
Conduct initial, semi-annual, and annual assessments, implement care plans
Assist with discharge planning for acute and long-term placements
Evaluate and treat participants during acute illness, prescribe medications/therapies
Make nursing home or home visits as needed
Help develop clinical policies, procedures, and standards of care
Train and support staff on medical and quality assurance issues

Requirements:
Master s degree in nursing REQUIRED
Active PA License REQUIRED
BLS, CDS, DEA REQUIRED
Board Certified by ANCC or AANP REQUIRED
