Job Title: Adult / Gerontology Nurse Practitioner

Location: New York, NY

Shift: Full & Part-time available

Responsibilities:

• Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident
• Order & interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning
• Participate in care initiation for our chronic care management services.

Requirements:

• Master's degree in nursing and graduate of an accredited Nurse Practitioner program
• Unrestricted New York state license as an ANP or AGNP
• Unrestricted DEA Certification
• Experience working with geriatric population and/or long-term care.
• Eligible to provide reimbursable services and maintain provided status with Medicare and Medicaid
• Minimum 3 years of experience; preferably in long-term care
• Experience with eClinicalWorks (eCW) electronic health records
