We re hiring a full-time advanced practice clinician to join a growing post-acute care team in the Brooklyn, New York area.
This position focuses on delivering high-quality, compassionate cardiology consults to adult patients in skilled nursing settings. With a Monday through Friday schedule and no nights or weekends, this role supports both your professional goals and your personal well-being.
What You ll Do:
Perform cardiology consults in post-acute care environments
Monitor and manage chronic cardiac conditions in collaboration with facility teams
Work closely with supervising physicians and interdisciplinary staff
Contribute to quality improvement efforts and patient safety initiatives
Document using a mobile EMR system, with full training and technical support provided
Ideal Candidate:
Certified nurse practitioner with an active New York license
Active DEA registration (or ability to obtain)
At least 2 years of experience in cardiology or post-acute care preferred
Compassionate and collaborative, with a patient-first mindset
Open to additional cardiology training if needed
