This position is for a Locum Geriatrics Physician in West Springfield, MA, focusing on providing comprehensive medical care to elderly participants under the PACE model. Responsibilities include conducting assessments, developing treatment plans, collaborating with interdisciplinary teams, and educating patients and families about chronic disease management. Candidates must hold a valid physician license in Massachusetts, have board certification in Internal Medicine or Family Practice, and preferably have geriatrics experience.

Job Responsibilities:- Conduct history and physical assessments on new participants alongside event-based and periodic reassessments.
- Develop medical treatment plans and provide necessary services across different participant locations.
- Interview and evaluate applicants to the PACE Organization and their caregivers, participating in Intake and Assessment decisions.
- Discuss medical conditions and prognoses with participants and their caregivers to aid in future care planning.
- Collaborate on the nursing treatment program for participants.
- Participate as an active member of the interdisciplinary team, educating on diagnoses, treatment options, and their implications.
- Engage in family conferences and decision-making processes.
- Educate participants and their families on health maintenance and chronic medical conditions.
- Supervise the development and implementation of the healthcare treatment program as designed by the interdisciplinary team.
- Conduct physical examinations and manage laboratory and diagnostic testing.
- Participate in provider on-call rotation as required.
- Effectively manage chronic disease treatment for frail older adults while adapting to changing participant needs.
- Current and active plenary physician license to practice in Massachusetts REQUIRED
- Board Certification in Internal Medicine or Family Practice REQUIRED
- Board Certification in Geriatrics PREFERRED
- Experience or added qualifications in geriatrics PREFERRED
- DEA Registration REQUIRED
- Previous supervisory experience REQUIRED
- Minimum of one (1) year of documented experience with a frail or elderly population REQUIRED
- Copy of NPDB self-query report REQUIRED
- Active Physician License in Massachusetts
- Board Certification in Internal Medicine or Family Practice
- DEA Registration
