- Contract Length:13 weeks
- Schedule:Monday–Friday
- Shift:5x8 Days | 8:00 AM – 4:30 PM (flexible start time)
- Expected Hours:40 hours/week
- Visits Per Day: ~4 visits/day
- Patient Population: Adults & Geriatrics
Setting & Patient Mix- Community-based home health visits with office on campus
- Average daily census: 26
- Common diagnoses include:
- COPD, CHF
- Orthopedic procedures (joint replacements)
- Neurological conditions (stroke, Parkinson’s, dementia, MS)
- Interdisciplinary support includes Social Services & Interpretation Services
- Reliable vehicle required
Requirements- License:
- Active Alaska PT License required (Compact PT accepted)
- Pending license NOT accepted
- Experience:
- Minimum 1 year Home Health / OASIS experience REQUIRED
- Epic experience preferred
- Certifications:
- Vaccines:
- COVID required (medical/religious exemptions allowed)
- Flu required – no exemptions
- Other:
- First-time travelers accepted if OASIS experienced
- No locals (60-mile radius rule)
- Must be separated from PeaceHealth for 1 year if previous employee
Floating- May float within scope, including Hospice if needed
- No floating to sister facilities
