The Cardiology Nurse Practitioner at Moxie Health Group LLC provides specialized cardiovascular care to patients in skilled nursing and long-term care facilities, focusing on reducing hospital readmissions and optimizing patient outcomes. Responsibilities include performing health assessments, developing treatment plans, prescribing medications, coordinating care, and maintaining thorough documentation. The role requires strong clinical skills, knowledge of post-acute and geriatric care, and experience with EMR/EHR systems in a multidisciplinary healthcare environment.
The Moxie Health Group LLC is a multi-specialty practice group specializing in pulmonary, infectious disease, cardiology, and post-acute care. We currently have over 100 providers delivering specialized care to patients in skilled nursing facilities across Florida, Ohio, Tennessee, and now in Pennsylvania and Texas.
A core aspect of our program is reducing hospital readmissions, ensuring responsible stewardship, and optimizing patient care within these facilities. Our Corporate Headquarters is in Hallandale Beach Florida.

Job Responsibilities
- Deliver direct patient care in skilled nursing, rehab, and long-term care settings
- Perform health assessments and physical examinations
- Diagnose and develop treatment plans tailored to each patient
- Prescribe medications per Florida guidelines and clinical best practices
- Complete documentation and maintain accurate health records (EMR/EHR)
- Conduct admission and discharge evaluations
- Coordinate care with physicians, facility staff, and specialists
- Promote preventive health practices and counsel patients on healthy living
- Ensure continuity and quality of care with timely interventions
- Participate in facility rounds and contribute to reducing hospital readmissions
- Educate patients and families about treatment plans and care routines
- Comply with infection control standards and regulatory policies
Qualifications
- Active Florida APRN license
- DEA registration (preferred)
- BLS and ACLS certifications
- Active NPI
- Minimum of 3 years of nursing experience (Skilled Nursing preferred)
- Knowledge of post-acute care and geriatric best practices
- Strong communication, assessment, and leadership skills
- Experience with EMR/EHR systems (Point Click Care/ Matrix)
- Comfortable working independently in a fast-paced, clinical environment
Working Hours:
5 Days a week with weekend availability
