Join a growing clinic that is looking for a Family Medicine provider. You would be Responsible for the delivery of patient-centered, team-based care falling within the scope of practice. Works collaboratively with Nurse Practitioners, RNs, LVNs, Social Workers, Case Managers and Medical Assistants to provide physical and psychosocial assessments, diagnosis and management of chronic and acute health and illness needs. Assumes responsibility and accountability for the continuity of healthcare and health education regardless of the presence or absence of disease.
