In Hillsboro we are small enough that the nurses cover both. We staff 2 nurses and 1 CNA to cover the hospital patients, outpatients, and ER all at the same time. Typical hospital census is around 8, with 1 or 2 outpatients that come in per day, and the ER sees an average of 3-4 patients per day (not per shift).
