Key Responsibilities
- Provide skilled physical therapy services to home health patients.
- Conduct patient assessments and develop individualized treatment plans.
- Deliver therapy sessions in patients' homes, ensuring proper documentation and compliance.
- Collaborate with caregivers and healthcare teams to optimize patient outcomes.
- Manage a caseload of approximately 4-5 patients daily, maintaining productivity standards.
Required Qualifications
- At least 1 year of recent, hands-on skilled home health experience (non-inpatient).
- Valid Washington state license or a compact license.
- BLS certification.
- Expert-level knowledge of OASIS documentation.
- Reliable personal transportation for travel within the territory.
Preferred Experience / Skills
- Previous home health therapy experience within the last 3 years.
- Ability to efficiently manage travel routes to minimize drive time.
