The Hospice/Palliative Medicine Nurse Practitioner provides in-home palliative care services to patients with serious illnesses, managing symptom relief and prescribing necessary treatments. This role supports a census of approximately 20 patients with an average daily volume of 4-6, delivering care within the community. The position requires a board-certified Family Nurse Practitioner with at least four years of experience, working Monday to Friday in Snellville, GA, and surrounding areas.

Additional Info: This role will hold a census of approximately 20 patients and provide Palliative Care services directly to patients in the community (in their homes). Services will include symptom management and prescribing to manage symptoms of serious illness. This NP will be solely responsible for providing and managing the care of these patients.
