Job Requirements

• Education Requirements: Bachelor’s degree in Physical Therapy or a related field from an accredited institution
• Certifications Requirements: State licensure as a Physical Therapist, CPR certification, Basic Life Support (BLS)
• Experience Requirements: Minimum of 1 year of home health experience, experience working with EMR systems, familiarity with productivity standards of 5-6 visits per day
• Other Requirements: Ability to work independently, strong communication skills, reliable transportation within a 30-mile radius, ability to manage a flexible schedule, excellent clinical skills

Job Responsibilities

• Provide comprehensive home health physical therapy services to patients in accordance with physician orders and patient treatment plans
• Assess patient needs, develop individualized treatment plans, and document progress accurately within the EMR system (Matrix)
• Educate patients and caregivers on therapy techniques, injury prevention, and health maintenance strategies
• Coordinate with other healthcare providers to ensure seamless patient care and optimal outcomes
• Manage a productivity schedule of 5-6 visits per day, ensuring timely and efficient service delivery
