The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.

Minimum Requirements:
Education: Diploma in Practical Nursing
Licensure: Active LPN licensure
