The Hospice Team Physician provides clinical and administrative oversight for hospice patients, ensuring compliance with Medicare regulations and high standards of end-of-life care. Responsibilities include certifying patient eligibility, reviewing treatment plans, collaborating with interdisciplinary teams, and conducting home visits. The role also involves educating staff, managing documentation, and serving as the physician designee when needed.
- Serve as the physician designee (as defined in 42 CFR §418.3) when the Medical Director is unavailable, according to a mutually agreed schedule.
- Be available to hospice staff for clinical guidance and patient care needs.
- Review and document clinical information for each hospice patient and certify life expectancy of six months or less, considering:
- Primary terminal condition
- Related and unrelated diagnoses
- Subjective and objective medical findings
- Medication regimen (hospice-related/covered vs. non-covered)
- Recommended treatment interventions and appropriateness
- Collaborate with the attending physician and interdisciplinary team to:
- Establish and review the plan of care at intervals no greater than 15 days
- Determine continued eligibility before each recertification period (§ 418.21(a))
- Participate in case conference meetings as requested
- Work with the Utilization Review Department to identify appropriate ICD-10 codes.
- Review and sign C2 (Controlled Drugs) prescriptions and death certificates, ensuring legal compliance and accuracy.
- Make home visits with hospice staff for clinical guidance and care plan adjustments.
- Facilitate communication between medical and hospice staff as needed.
- May assume attending physician role for General In-Patient level of care in hospice homes and Sharp Hospitals (with privileges), though these services are not billed as administrative services.
- Document in the Electronic Health Record per agency protocol.
- Perform face-to-face encounter visits per Medicare and hospice policy.
- Educate hospice staff through formal and informal in-services.
