This position is for a board-certified Family Practice physician without OB duties, focusing on leading a team of Nurse Practitioners and managing a patient panel. The role offers a Monday to Friday schedule with no weekends, emphasizing collaboration, direct patient care, and home-based care models. Opportunity Highlights:
- Monday-Friday schedule: 8:00 AM to 4:30 PM, no weekends
- Weekly high-risk huddles with your advanced practice team
- 4 hours of dedicated collaboration time per NP per week, including ride-alongs for patient visits
- Remaining weekdays focused on direct patient care - 7 patient contact hours daily with Medical Assistant driver support (chart in transit, no after-hours admin work)
- Manage a patient panel of 150-170 individuals while leading 4 Nurse Practitioners
- Full-time providers carry an average panel of 275 patients
- Work alongside social workers, care managers, navigators, and community health partners
- Shared call, rotated weekly across all providers in the state (triage team takes first call)
- Dyad leadership partnership with your Market Manager
Compensation & Benefits:
- Base Salary: $260,000-$290,000, with $290K targeted for these roles
- Annual Bonus: $40,000-$60,000, based on productivity, quality metrics, and team performance
- Retention Bonus: $75,000, paid over three years
