Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Ohio Physical Therapy license required to start the assignment.
  • Current BLS/CPR certification from American Heart Association or American Red Cross required.
  • Valid driver's license, current vehicle insurance, and access to a dependable vehicle for home visits.
  • Experience delivering home health physical therapy services, including evaluation, plan of care development, progress documentation, and discharge summaries.
  • Ability to perform full patient evaluations and develop PT plans of care within state guidelines and report outcomes within required timeframes.
  • Ability to supervise PTAs and home health aides as allowed by state regulation and agency policy.
  • Schedule flexibility to participate in weekend rotations and potential availability on observed holidays.
  • Maintain required state licensure and participate in continuing education to remain current with standards of care.
  • Preferred: prior travel or agency home health experience and familiarity with CMS documentation requirements.


Assignment Details

  • Facility Type: Home Health.
  • Position Type: Travel Physical Therapist, Full Time.
  • Responsibilities include patient evaluations, development and implementation of PT plan of care, progress notes per agency policy, and preparation of written discharge/summary reports at least every 60 days.
  • Visit patients across the agency service area and may provide coverage to nearby agency service areas as needed (floating required).
  • Expected to consult with physicians, coordinate with interdisciplinary team members, provide in-service education as requested, and adhere to agency therapy standards of practice.
