UChicago Medicine AdventHealth is hiring for their Community Palliative Care program. This is a growing program which is so needed in our communities. The program serves our homebound patients, giving them access to palliative care in the comfort of their homes.

1-3 years of APP experience, preferred

Palliative Care Certification, preferred

Weekly Monday - Friday schedule

No call requirement

Outpatient palliative care in patient s homes

Visit patients in assigned territory

Work collaboratively with Social Workers and House Calls Physicians & APP's

Annual Salary: $110K - $120K

REQUIRED QUALIFICATIONS:

Education/Training

• Graduate of an accredited school of nursing and nurse practitioner program
• Masters degree in Nursing or Physician Assistant Studies
• Current Illinois license in good standing
• Certified as a nurse practitioner or physician assistant by an approved certifying organization.
• Prescriptive authority, Illinois Controlled Substance, DEA certification
• Current CPR certification
