Schedule: Full Time/ Days

Supports Damascus, Sandy, Boring and Welches

Manages and provides skilled occupational therapy to patients and clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician directed plan of treatment, taking into account individual patients special or age related needs.

Required qualifications:

• Bachelor's Degree From an accredited Occupational Therapy Program Or;
• Master's Degree From an accredited Occupational Therapy Program Or;
• Ph.D. From an accredited Occupational Therapy Program.
• Within 30 days of hire: National Provider BLS - American Heart Association.
• Upon hire: Oregon Occupational Therapist License.
• Washington Occupational Therapist License (Vendor Managed) - if practicing within this state is required upon hire.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
