The Geriatrics Nurse Practitioner provides comprehensive, value-based primary care to Medicare patients, focusing on geriatric health and coordinating with care teams to manage patient wellness and chronic conditions. Responsibilities include conducting routine and acute office visits, administering wellness visits and health risk assessments, and performing home visits as needed. The role requires advanced nursing certification, experience in primary care specialties, and familiarity with clinical documentation and coding standards.
Details:
- Full-time, Mon - Fri, 8am-5pm
- 13-16 patients per day
- Geriatric population
- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)
- Assisting the care team with phone triage and outreach.
- Conducting home visits as needed
Qualifications:
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate required
- National certification as a Family, Adult or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
- Experience in primary care: Internal medicine, geriatrics, or family medicine preferred
- Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
