A mission-driven nonprofit organization is seeking a dedicated and compassionate Psychiatric Nurse Practitioner to join a multidisciplinary team providing outpatient mental health services to individuals impacted by complex life circumstances, including involvement with public systems. The organization is committed to advancing recovery, reducing barriers to care, and promoting long-term wellness in underserved communities.
The Psychiatric Nurse Practitioner will play a key role in delivering integrated, trauma-informed psychiatric care in a community-based setting. This position involves conducting psychiatric evaluations, managing medications, and collaborating with a team of clinicians to support individualized treatment plans.
Key Responsibilities:
- Conduct psychiatric assessments and follow-up visits for clients across the lifespan.
- Prescribe and manage psychotropic medications in alignment with clinical guidelines.
- Collaborate with therapists, case managers, and medical staff to support holistic care.
- Provide education to clients and families about diagnoses, treatment options, and medication side effects.
- Respond to urgent client needs and participate in risk assessments.
- Support integrated care efforts, including coordination with primary care and substance use services.
- Maintain accurate and timely clinical documentation in accordance with regulatory standards.
Requirements:
- Licensed and certified Psychiatric Nurse Practitioner (PMHNP) in New York State.
- Experience working with individuals with serious mental illness and/or co-occurring disorders.
- Strong commitment to recovery-oriented, culturally responsive care.
- Ability to work effectively in a team-based, fast-paced environment.
