Key Responsibilities:
- Provide one-on-one therapy sessions in patients' home settings
- Collaborate with interdisciplinary teams and colleagues to ensure comprehensive patient care
- Perform standard therapy measurement techniques, including range of motion (ROM), strength, and balance assessments.
Required Qualifications:
- Valid Vermont Physical Therapist licensure
- Home Health experience is a must.
- OASIS experience required, as this role involves case management.
- Strong familiarity with McKesson/Netsmart EMR systems.
Preferred Experience / Skills
- Experience in home health care settings
- Prior experience with therapy measurement tools and modalities.
