Primary Care Physician OpportunityLocation: Rutland, VT - Manchester, NH 120m; Albany, NY 90mSetting: Multi-site Community Health / FQHCEmployment Type: Full-Time, EmployedSchedule: 4-day work week with flexible hoursPosition Overview Outpatient primary care role serving a diverse patient population Practice within a multi-site primary care system across central Vermont Opportunity to provide comprehensive, longitudinal care focused on prevention and wellness Minimal after-hours call shared across sites (triage only) Supportive team-based care model with strong clinical infrastructureClinical Responsibilities Provide comprehensive primary care services to patients of all ages Promote preventive care, wellness, and chronic disease management Empower patients to actively participate in their health care Provide appropriate follow-up and clear patient communication Refer patients to specialty care as clinically indicated Provide clinical supervision to advanced practice providers and nursing staff Participate in quality assurance and quality improvement initiatives Engage in state and national disease collaborative effortsPractice Highlights Certified patient-centered medical home model Robust panel management and case management support Integrated behavioral health services embedded within primary care Fully integrated electronic medical record and practice management system Strong collaboration with local hospital servicesQualifications MD or DO Board certification strongly preferred Eligible for licensure in the state of Vermont Completion of accredited medical education and residency trainingCompensation & Benefits Competitive salary commensurate with experience Retirement plan with employer match Up to 6 weeks of vacation Paid malpractice coverage Relocation assistance up to $10,000 CME allowance of $5,000 with one week of CME time School loan repayment assistanceCommunity Located in a scenic New England region with four-season outdoor recreation Access to hiking, skiing, biking, and lakes Safe, welcoming communities with a strong quality of life Easy access to larger metropolitan areas in the Northeast
