This role is for a board-certified Hospice/Palliative Medicine Physician who provides compassionate care to patients with chronic complex conditions through a combination of in-home visits and telemedicine. The physician collaborates with interdisciplinary teams to manage symptoms, coordinate referrals, and improve patient quality of life in a community-based setting. The position offers a hybrid schedule and focuses on serving diverse populations, including Medicare, Medicaid, and commercial patients.
A leading national healthcare organization is seeking a full-time, board-certified Palliative Care Specialist to join its growing team. This physician will provide compassionate, patient-centered care to individuals with complex chronic conditions such as CKD/ESRD, COPD, and CHF. The role combines in-home visits with telemedicine, offering a hybrid schedule and the opportunity to make a meaningful impact in your local community.
The Opportunity:
- Deliver palliative care services to patients in the community through home visits and tele-health
- Manage symptoms affecting quality of life coordinate referrals, educate care teams and track clinical and financial outcomes in collaboration with Medical Directors and interdisciplinary teams

- Fellowship training and BC/BE in Hospice and Palliative Medicine

- Comfort and skilled in care discussions, advance care planning, symptom management, specifically complex medication panels, acute and behavioral health care and appreciation of social determinants
- Licensure or ability to become licensed in required state(s)
- Must have reliable transportation
