We are seeking a Family Nurse Practitioner for a 13-week locum tenens assignment in Illinois. This opportunity begins on Dec 15, 2025, and features a Monday through Friday, 9am to 5pm schedule. The assignment involves travel within an hour of your home area to provide patient care at contracted assisted living facilities.

Responsibilities and Duties- Meet with residents and patients at contracted assisted living facilities to conduct initial patient visits
- Establish provider-patient relationships and perform physical assessments, including listening to lungs and heart
- Log patient interactions and findings into the OmniCare EMR system for tracking by the clinical team
- Travel to facilities within an hour from your home area as needed
Additional Information- Must be licensed as a Nurse Practitioner in Illinois
- Willingness to travel up to an hour outside of your home area is required
