Position Highlights:

• Deliver primary and urgent care in patients homes or via telehealth

• Work with a supportive, multidisciplinary team focused on patient-centered outcomes

• Manage chronic conditions and provide holistic, high-touch care

• Participate in care transitions, social determinant interventions, and complex care coordination

• Ongoing clinical education and support from experts in geriatrics, psychiatry, pharmacy, and palliative care

Schedule:

• Flexible care delivery (home visits & virtual)

• After-hours call coverage rotation required

Requirements:

• Board-certified NP or PA with active Washington license

• Active DEA and BLS certification

• At least 5 years of licensed driving history with clean driving record

• Must be able to travel within region as needed
