Our clinics offer a full range of primary care, including chronic disease management, pediatric care, childhood immunizations and obstetrical care.Hospital is a 79-bed acute care facility located in historic downtown Albany, Oregon. Rich in history, the current hospital is built in the same location as first Albany hospital and is a level III trauma center providing medical services to the greater Albany, Oregon area.Our award-winning health system provides exceptional primary care and a range of specialty services throughout the mid-Willamette Valley and Central Oregon coast. The network serves approximately 300,000 residents in Linn, Benton, Lincoln and portions of Polk and Marion counties. Our skilled providers have a passion for building healthier communities through world-class, compassionate health care.Job Specifics:-16-18 patients per day, with longer scheduled time to see patients to increase quality of care.-Two care teams made up of 1 MD/DO, 2 APCs, and MA, Behavioral Health, Psychiatry, Clinical Pharmacist, RN, Care Coordinator, Community Health Worker, and Front Office support.-Typical cases/patients are adult IHN patients, typically with multiple diagnosis and complex, with a mix of healthy adult patients.
