The Role
As a Primary Care Nurse Practitioner, you ll practice in a value-based care model focused on quality over volume. You ll manage a patient panel of older adults living with chronic and complex conditions, supported by a dedicated care team.
Practice Highlights:
100% outpatient practice
Patient population: seniors with 3 5 chronic conditions (e.g., heart disease, diabetes, hypertension)
Average of patients per day
Patient panel capped at 650
Dedicated support team including MA, scheduler, and referral specialist
