Our job summary

This position is for a Home Health Physical Therapist in Globe, Arizona, providing personalized physical therapy services to patients in their homes. Responsibilities include assessing patient status, modifying treatment plans, documenting interventions, and educating patients and caregivers. The role requires a current Physical Therapist license, experience in physical therapy, and certifications such as CPR and clearance cards.

Compensation: $70 - $95 per visit depending on experience, PLUS millage and travel time

As a Home Health Physical Therapist, here’s a big-picture view of what you’ll do:

Complete initial and ongoing comprehensive assessments at appropriate time points.
Assess changes in the patient's status since the last visit/treatment day.
Determine whether the planned services should be modified.
Submit documentation of treatment and skilled intervention provided.
Participate in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.

A few must-haves for Home Health Physical Therapists:

Current licensure or certification as a Physical Therapist active in Arizona.
One (1) year Physical Therapy experience.
Experience working in home care setting preferred.
CPR Certification (demonstration course required)
Valid Arizona Fingerprint Clearance card.
No Covid Vaccination Required
