This is a leadership role focused on delivering high-quality, coordinated care for seniors with chronic conditions, emphasizing independence, wellness, and dignity.
You will oversee clinical operations, guide interdisciplinary care teams, supervise providers, and ensure best practices in geriatric medicine are upheld.
Key Responsibilities Conduct H&P exams for new and established participants
Develop and implement medical treatment plans
Participate in intake evaluations and care planning
Provide medical guidance to participants and caregivers
Collaborate with nursing and interdisciplinary teams
Oversee QAPI and clinical quality standards
Supervise PCPs and APPs
Participate in family conferences and team meetings
Manage chronic disease care for frail older adults
Participate in shared on-call rotation
Maintain medical policy standards and protocols
- Full-time day schedule
- Community-based elder care program
- Team includes PCPs, APPs, social workers, therapists & support staff
- EMR: Not specified but fully integrated care model
- Prior supervisory leadership required
- Must have experience working with frail or elderly populations
- Administrative support provided by Executive Director
- Active Massachusetts medical license
- Board Certified: Internal Medicine or Family Medicine
- Geriatrics Board Certification or geriatric experience preferred
- CDS Certification & DEA required
- Minimum 1 year experience with frail or elderly patients
- Strong skills in chronic disease management
- Proven ability to supervise clinical staff
- Must be able to manage evolving priorities
