Levindale Hebrew Geriatric Center & Hospital – Subacute Rehabilitation (Hall 1)

Unit Summary

Hall 1 is a 42-bed Subacute Rehabilitation unit within Levindale, providing long-term care residents with daily activities and a comprehensive range of clinical services and support. This unit cares for patients with highly complex medical needs in a fast-paced environment. Patient populations include individuals recovering from post-operative procedures, strokes, cardiac events, and those requiring ambulatory rehabilitation, as well as extensive wound care management and prevention.

Job Summary

The Registered Nurse (RN) provides safe, compassionate, and age-appropriate nursing care to residents in the Subacute Rehabilitation unit. The RN functions as a clinical resource and advisor to patients and their families while collaborating with the interdisciplinary care team to deliver high-quality outcomes. This role works under the supervision of the Nurse Manager and plays a critical role in managing complex clinical needs in a dynamic care environment.

Essential Responsibilities
  • Provide comprehensive nursing care to residents with complex medical and rehabilitation needs

  • Perform assessments, develop and implement individualized plans of care, and evaluate patient outcomes

  • Administer medications and treatments in accordance with physician orders and facility protocols

  • Provide wound care management, prevention strategies, and documentation

  • Monitor patient conditions and respond appropriately to changes in status

  • Educate patients and families regarding treatment plans, medications, and ongoing care needs

  • Collaborate effectively with physicians, therapists, and interdisciplinary team members

  • Ensure compliance with regulatory standards, infection control practices, and patient safety initiatives

  • Serve as a clinical resource and mentor to LPNs and CNAs as appropriate

Requirements
  • Formal working knowledge equivalent to an Associate’s Degree in Nursing

  • Bachelor of Science in Nursing (BSN) from a CCNE-accredited school preferred

  • 1–3 years of Registered Nurse experience required; subacute, rehab, or LTC experience preferred

  • Active Maryland Registered Nurse (RN) License

  • American Heart Association BLS Certification required

  • ACLS certification is required within one (1) year of hire.
