Physical Therapist - Home Health - (PT - HH)
StartDate: ASAP Available Shifts: 10 D Pay Rate: $1789.00 - $1844.00

Tell us about the unit(s): · Number of Beds – approximately 70 patients that are seen by any combination of disciplines · Number of Staff – 25 (including nursing, rehab, office, and administrative staff) · Type of staff: Works with other PT’s, PTA’s, OT, RN’s, CNA’s, SW. This service is carried out where the patient calls home: personal home, ALF o Day Shift – Office open 8-4:30 Monday-Friday (Nurse on call 24/7) o Night Shift - NA · Patient Ratios – 6 visit equivalents per day ( types of visits are weighted differently on a point system) · Type of equipment – · EMR - Cerner Typical hiring profile Skill Set Most recruited for: Home Health Must have: candidates without these skills will not be considered for the role. · Experience in completing OASIS Assessments · Home Health experience Preferred or nice to have: candidates with these skills will be considered first. · Some knowledge of new payment model coming 1-1-20 (PDGM) · List typical procedures performed on unit(s): · Comprehensive assessments/evaluations, treatment and care planning. Oversight of PTA’s. · Patient population: mainly geriatric; post-acute treatment of THA/TKA patients; treatment of patients with chronic illnesses and multiple comorbidities. Best personality Fit: · Someone who is easy going, likes to meet people and doesn’t mind visiting people in their own homes. Needs to be accepting of others and friendly. REQUIRED CERTIFICATIONS: BLS/CPR certification. Needs current NC license to practice Physical Therapy Floating Area(s): · none Dress Code: · Not required to wear scrubs. Business casual, closed toed shoes. Unit Guidelines/Policies: Please feel free to attach unit policies and guidelines. · Will be discussed in interview. Weekends and On call(if any) · Weekends only as needed, not as a routine expectation
