Post Acute Care Physician opportunity in Media, PA
Great full-time (5 days per week) primary care role, you will have the opportunity to manage complex patient cases in post-acute settings (skilled nursing and independent living facilities) in Media, Pennsylvania, focusing on improving care for some of the most vulnerable patients.

- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Provide oversight, guidance, and mentorship to our nurse practitioners
- Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
Qualifications:- Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered care
- Post-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunities
- Active and unrestricted Pennsylvania license (or in process)
- Active and unrestricted DEA/prescriptive authority (or in process)
- Internal medicine (IM), family medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are welcome to apply
Compensation:- Compensation is fee for service (FFS). Expected compensation is an estimated range of $180,000 to $291,000 annually with no cap on productivity income potential.
