Seeking a Nurse Practitioner to join our Skilled Nursing Facility Service Line in Rutland, Vermont!
Details:
- Provide primary healthcare to our geriatric population residing in Skilled Nursing Facilities in the greater Rutland Area
- Our SNF Nurse Practitioners perform selective medical services through a collaborative agreement with a supervising MD/DO
- Must enjoy working with a dedicated team of highly motivated professionals who are committed to providing an integrated, community-based approach to care

Essential functions include though are not limited to:
- Obtains complete medical history and physical examination on patients
- Reviews medical records
- Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures
- Establishes presumptive diagnosis
- Orders appropriate laboratory and diagnostic procedures
- Develops a therapeutic plan for patients
- Provides orders for drugs and controlled substances
- Serves as the resource go to Provider for designated SNF
- Collaborates and coordinates care with SNF Care Coordinators
- Works collaboratively with other community partners/Providers to optimize patient care
- Assists in the development and monitoring of Quality Improvement for SNF residents
- Participates in on call coverage for SNF Service Line
- Provides ongoing continuity of care
