Responsibilities
- Provide medical leadership and oversight of the interdisciplinary clinical team in a PACE (Program of All-Inclusive Care for the Elderly) setting
- Conduct initial and periodic assessments of participants, develop and manage medical treatment plans
- Participate in intake evaluations and eligibility determinations
- Lead quality assurance and performance improvement (QAPI) initiatives
- Supervise and collaborate with primary care and advanced practice providers
- Oversee medical policies, procedures, and protocols to ensure quality care
- Educate participants and their caregivers on chronic conditions and care plans
- Participate in on-call provider rotation as required
- Foster a team-based, participant-centered care environment
Requirements
- Clear and active medical license in the State of Florida
- Board Certification in Internal Medicine or Family Medicine (required)
- Board Certification or added qualifications in Geriatric Medicine (preferred)
- Minimum of one year experience caring for a frail or elderly population (required)
- Demonstrated leadership or supervisory experience (required)
- CDS Certification and DEA Registration (required)
- Strong skills in physical assessment and chronic disease management for older adults
- Passion for working in an innovative, community-based healthcare setting
- Ability to adapt to changing priorities and patient needs
