A healthcare system in Vermont is currently a licensed Physician, with a Family and/or Internal Medicine background, for a promisingopportunity at one of their facilities in Burlington.

About the Opportunity:

• Start Date:ASAP
• Schedule: Monday to Friday
• Hours: 8am to 5pm
• Setting: Skilled Nursing Facility / Clinic
• Specialty:Family / Internal Medicine
• Patient Population:Geriatric
• Patient Volume: 10 to 16 per day
• EMR:Epic

Responsibilities:

The Family / Internal Medicine Physician will be responsible for:

• Seeing patients for preventative visits
• Assisting patients withacute and chronic ongoing needs
• Handling SNF admissions anddischarges
• Doing periodic assessments
• Performing other duties, as needed

Qualifications:

• Licensed to practice medicine in the State of Vermont
• Board Certified / Eligible in Family and/or InternalMedicine
• Experience working with the Geriatric population
• Solid assessment, clinical, and documentation skills
• Great interpersonal skills
• Excellent communicationskills (written and verbal)
