A hospital in Staten Island is looking for a Geriatrician to join their dedicated outpatient team.

About the Opportunity:

• Schedule: Monday-Friday
• Hours: 9am-5pm
• Setting: Outpatient
• Patient Population: Adults 65+

Responsibilities:

• Provide office-based management of the geriatric population
• Impart geriatric care protocols and standards across all network adult medicine providers
• Teach medical residents the principals of geriatric care, including giving lectures
• Create continuum of care transition models to support home health, family support, and post-acute care protocols
• Work collaboratively with hospital medicine and ED physicians to create a well-developed program of competencies in managing geriatric patient needs

Qualifications:

• License to practice medicine in New York
• Board Certified/Eligible in Internal Medicine or Family Medicine AND the completion of a geriatrics fellowship OR a passion for working with geriatric patients
