Their focus is on community medicine, cultural competency, and patient outcomes, supported by behavioral health, complex care, lifestyle medicine, and remote patient monitoring. They are seeking passionate Primary Care Physicians to join their team across multiple locations in Cape Cod.
As a Primary Care Physician with their team, you ll have the opportunity to:
Manage a diverse patient panel in a collaborative, team-based care model
Conduct both in-office and tele-health visits
Collaborate with a multidisciplinary team including behavioral health, complex care, lifestyle medicine, and remote patient monitoring
Supervise NP colleagues
Lead population health initiatives and quality assurance programs
Qualifications:
Board Certified in Internal Medicine or Family Medicine, or Board Eligible with plans to obtain certification within 1 year
Graduate of an accredited medical school and residency program in Family Medicine/Internal Medicine
Massachusetts medical license (or ability to obtain before start date)
