Responsibilities and Duties
- Deliver comprehensive care to geriatric patients in a community health center setting
- Focus on chronic care management and preventative medicine to minimize hospitalizations
- Take time with patients to ensure high-quality, individualized care
- Accurately code and chart patient encounters
- Utilize technology effectively for patient records and communication
- Maintain an organized workflow in a fast-paced environment
Additional Information
- Active Florida medical license required
- Board certification in Family Medicine, Geriatric Medicine, or Internal Medicine required
- Basic Life Support (BLS) certification required
- 2-3 years of post-residency experience, including work with chronic care in community health centers
- Experience caring for geriatric patients strongly preferred
