Seeking a full-time Nurse Practitioner to join an interdisciplinary care team providing community-based care to frail and elderly adults. The focus is on supporting independence, dignity, and quality of life while delivering the full continuum of medical and supportive services.
- Full-time weekday schedule
- Patient population: Adults 55+
- All participants are certified to require nursing home-level care but primarily reside in the community
- Program coordinates and covers care if nursing home placement becomes necessary
- Perform comprehensive medical assessments
- Develop and implement individualized care plans
- Evaluate and treat acute illnesses
- Participate in discharge planning
- Conduct home visits as clinically indicated
- Coordinate care across the full continuum of services
- Assist with development and review of clinical policies and procedures
- Participate in provider on-call rotation
- Board Certified Nurse Practitioner (ANCC or AANP)
- Active Massachusetts Nurse Practitioner license
- DEA Registration
- Controlled Dangerous Substances (CDS) certification
- BLS certification
- Experience working with frail or elderly populations
- Ability to travel within the service area
- Interest in community-based, mission-driven care for seniors
