Position Summary/Department Summary:Per Diem Social Worker needed for the Boston Children’s Hospital Primary Care Center in Weymouth – free parking!
Key Responsibilities:- Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.
- Performing psychosocial and risk assessments, assigning diagnoses, and developing and implementing interventions and treatment plans.
- Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan
- Implementing and following up on treatment and provision of needed services.
Minimum Qualifications
Education:- Master’s degree in social work
Experience:- Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
- Experience with social services agencies, understanding of social determinants of health and trauma ideal.
- Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!
Licensure/Certifications:- Active LICSW in the state of Massachusetts
The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.
