Details:

• Setting: hybrid remote and in-patients' homes role
• Location: Bloomsburg, PA 17815
• Schedule: Mon-Fri (8a-5p includes travel time for in-home visits)
• Duties:
• APP's serve multiple serious conditions like CKD and/or ESRD/ESKD
• Conducts assessments including annual wellness exams
• Counsels and educates patients and their families about programs
• Responsible for coordination of care with primary care providers
• Part of the Integrated care team working with RN and Social Worker
• Prescribe medications, order tests, collaborate with Monogram physician
• Follow up and evaluate patient's status

Required :

• NP, FNP, AGNP, ACAGNP with Active PA license
• Board Certified- ANCC or AANP
• 2 or more years of direct patient care
• DEA, BLS

Compensation :

• Discussed by phone
• Receive a paycheck every Friday
• Mileage reimburseed
