• Banner requires a specific cover page — all fields must be 100% accurate • Vet all locations with the traveler and list all departments they are willing to accept/work in to speed up presentation Submission Requirements: • Start date must be on or very close to the order start date • All RTO must be listed on the cover page (max < 7 days) • No RTO allowed during the first week or on holidays • List all facilities the traveler is open to working at • If open to multiple locations, specify states (e.g., Any AZ, Any CO, Any State) • Include all shifts the traveler is willing to work • All acceptable departments must be checked on the cover page • Candidates must be pre-closed and ready to accept • Notify immediately if a traveler withdraws or becomes unavailable
