Title: Physical Therapist – Home Health
Location: Sturtevant, WI
Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Job Responsibilities and Required/Preferred Skills:
Provide home health physical therapy services, primarily for geriatric patients.
Complete patient assessments, evaluations, plans of care, OASIS documentation, and timely follow-ups.
Maintain all documentation by 9 AM the following day and communicate effectively with the team.
Manage a patient load averaging 5–7 visits per day, meeting minimum productivity requirements (30 points weekly).
Travel within a 60-mile radius of the hub/office as needed.
Participate in on-call rotation every 3rd to 4th weekend.
Use HomeCare Homebase (HCHB) EMR for charting and documentation.
Follow facility dress code (scrubs, no jeans) and utilize iPad for patient care documentation.
Mileage reimbursed at IRS standard rate.
Required Skills and Qualifications:
Minimum 1 year of recent physical therapy experience.
Proficiency in OASIS and HCHB EMR.
Active PT license in-hand.
BLS certification.
Personal transportation and valid driver's license.
Ability to work independently in home health settings.
Preferred:
Previous home health experience.
Flexibility with territory and patient scheduling.
Experience managing timely communication and documentation in home health care.
