About the Role:
- Under the supervision of the unit leadership, the LPN utilizes a general understanding of the principles of nursing and basic physical assessment skills in the development and implementation of individualized nursing interdisciplinary plans to ensure that the needs of the patients are met; establishes collaborative relationships with the RN and PCA/CCT to ensure all daily operations of the unit are met.
KEY RESPONSIBILITIES:
- In a team relationship, conducts ongoing reassessment and gathers relevant clinical data of the patient or family.
- Performs ongoing focused systems assessment, as appropriate to specialty; excluding initial assessment and admission database.
- Performs daily assessments and observes patient's clinical condition.
- Communicates effectively to registered nurse or charge nurse about deteriorating patients (i. e. changes in vital signs, Rothman warning trends, and mental status).
- Reviews plan of care, completes necessary documentation for plan of care per shift.
- Assesses patient education needs, completes patient / family education and documents appropriately.
- Assists with patient safety rounding and communicates findings to the assigned RN.
- In collaboration with the RN identifies discharge planning needs of the patient/family/significant other on admission.
- Assists patients in performing activities of daily living while optimizing autonomy and independence.
- Performs technical skills including initiation of IVs, phlebotomy, EKGs, indwelling or straight catheter insertion, maintenance and removal of catheters and dressing changes using aseptic technique.
REQUIREMENTS:
- Education: HS Diploma/GED, Graduate of an accredited school of nursing
- Licensure/Certifications: Current Maryland license or eligibility to obtain Maryland license, AHA BLS CPR
- Experience: Prior LPN experience
