- (NYS) Patient-Centered Medical Home recognized through the National Committee for Quality Assurance (NCQA). The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement
- Join an established group of 1 full-time physician, 2 Advanced Practice Clinicians
- Traditional Monday Friday, 8 AM 5 PM
- Opportunity for 4/10 hour shifts, 7 AM 5 PM
- No OB
- A first call system is in place with a nurse practitioner/physician assistant on call for your patients
- Integrated addiction care with large supportive network of MOUD clinicians throughout our clinics.

- Salary Range: $234,321 - $270,325. Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards
