We are seeking a licensed Physical Therapist (PT) for a contract assignment in a Skilled Nursing Facility. The Physical Therapist will evaluate, plan, and implement individualized treatment programs to help residents improve mobility, strength, and functional independence.
Key Responsibilities- Conduct physical therapy evaluations and develop individualized treatment plans
- Provide therapeutic exercises, gait training, balance training, and functional mobility interventions
- Monitor patient progress and modify treatment plans as needed
- Collaborate with interdisciplinary teams to support patient recovery and discharge planning
- Educate patients and caregivers on mobility strategies and home exercise programs
- Maintain accurate, timely documentation in accordance with facility standards
- Active state Physical Therapist license (must be current)
- Experience in a Skilled Nursing Facility preferred
- Strong clinical, communication, and documentation skills
- Minimum experience: less than 1 year (newer clinicians encouraged to apply)
- No holiday coverage required
- Bilingual skills not required
