Position Details:
Patient Population: Elderly, frail individuals needing chronic care
Setting: Program of All-Inclusive Care for the Elderly (PACE)
Responsibilities: Primary medical care for elderly patients, assessments, development and implementation of care plans, collaboration with interdisciplinary team, referrals, and management of specialist care.
Required Procedures: Various medical procedures as indicated
EMR System: TBD
Support Staff: Interdisciplinary care team
Days Per Month: Full-time, Monday to Friday
On-Call/Weekends: Required for after-hours/weekend coverage on a rotating basis
Submission Requirements:
- Board Certified in Internal Medicine or Family Medicine - REQUIRED
- Board Certified in Geriatric Medicine - PREFERRED
- Active Delaware license - REQUIRED
- CSR/CDS Certification (prior to start) - REQUIRED
- DEA Registration - Verbal Confirmation (prior to start) - REQUIRED
- Availability to begin working and state interest in Permanent - REQUIRED
- Minimum 1 year working with the frail or elderly population - REQUIRED
- BLS (prior to start) - REQUIRED
- Clean/Clear malpractice - HIGHLY PREFERRED
- EMR TruChart experience - HIGHLY PREFERRED
