- 1 advanced practice provider (APP)
- Experienced support staff: CMAs, nurses, lab technicians, and a referral coordinator

- Completion of an accredited Family Medicine residency program
- Current MD or DO license (or eligibility) in North Carolina
- Board-certified or board-eligible in Family Medicine
