Home Health Physical Therapist (PT)
Job Summary:
Provide skilled physical therapy services to patients in their homes, promoting mobility, independence, and overall quality of life through personalized treatment plans and education.

Key Responsibilities:
- Conduct comprehensive physical therapy evaluations in patients’ homes.
- Develop individualized treatment plans based on physician orders, assessment findings, and patient goals.
- Deliver evidence-based therapeutic interventions to improve strength, balance, mobility, and functional independence.
- Educate patients, caregivers, and families on home exercise programs, safety techniques, and assistive device use.
- Monitor and document patient progress, response to treatment, and any changes in condition.
- Communicate regularly with physicians, nurses, and other interdisciplinary team members to coordinate care.
- Ensure compliance with agency policies, state practice acts, and federal regulations.
- Maintain accurate and timely clinical documentation according to agency and payer requirements.
- Provide input for discharge planning and recommend continuation of therapy or community resources as appropriate.
- Participate in case conferences, quality improvement activities, and ongoing professional development.
Qualifications:
- Licensed Physical Therapist (PT) in Michigan.
- CPR certification
- Minimum 1 years of clinical experience, preferably within home health in the last 5 years.
- Valid driver’s license, reliable transportation, and proof of insurance.
- Strong clinical reasoning, communication, and time-management skills.
- Compassionate, patient-centered approach with the ability to work independently.
